What are Dean's Certification Forms?
Dean's certification forms require information regarding an applicant's academic and non-academic conduct. Any incidents of academic or non-academic misconduct are reported on the forms. These forms are required by only a few law schools.
Who completes Dean's Certification Forms?
The KU Pre-Law Advisor completes dean certification forms for applicants in all majors. Inquiries are made to the Dean's Office of the school from which you will graduate and the Vice Provost for Student Success for information regarding academic and non-academic misconduct.
How do I request a Dean's Certification Form to be completed by the Pre-Law Advisor?
Please provide the following to the Pre-Law Advisor. (You may drop off this information at the front desk of the University Advising Center, 126 Strong Hall.)
- Dean's Certification Forms - complete and sign the student portion of each form.
- Current ARTS form
- Envelopes
- If you wish to have the forms sent directly to the law schools upon completion, please include stamped, addressed envelopes for each law school.
- If you wish to send the forms with your applications, please include envelopes addressed for each law school (stamps not required).
- E-mail address - you will be notified by e-mail when the process has been completed and the forms have been mailed or when your forms are ready to be picked up.
How long does it take to complete Dean's Certification Forms?
In completing dean’s certification forms, information regarding academic and non-academic misconduct is requested from the Dean's Office of the school from which you will graduate and the Office of the Vice Provost for Student Success. Please allow 2 weeks for completion of the forms.
